Dear Sirs,

 

DFM Sp. z o.o. with headquarters in Dobre Miasto, ul. Olsztyńska 21, 11-040 Dobre Miasto, Poland, pursuant to art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union of 2016 No. 119, page 1) – further on GDPR, DFM Sp. z o.o. kindly informs you that:

 

1)    the controller of your personal data is DFM Sp. z o.o. with headquarters in Dobre Miasto, ul. Olsztyńska 21, 11-040 Dobre Miasto, Poland, National Court Register (KRS) No. 0000000630 (hereinafter – DFM);

 

2)    data protection officer at DFM is Mrs. Barbara Kochanowicz, address as above, e-mail address: iod@dfm.com.pl ;

 

3)    your personal data will be processed for purposes related to conducting business activity by DFM, including for the purpose of maintaining trade and business contacts;

 

4)    recipients / categories of recipients of your personal data will be employees and co-operators of DFM, dealing with DFM business matters;

 

5)    your personal data will not, as a rule, be transferred to a third country;

 

6)    your personal data will be kept for the period necessary to maintain active business contact and to perform contracts, unless you withdraw your consent to the processing of personal data;

 

7)    legal basis for the processing of personal data:

       – art. 6 para. 1 GDPR;

 

8)    you have the right to access to your data and the right to rectify, erase, restrict processing your data, the right to transfer data, the right to lodge a complaint with a supervisory authority, the right to withdraw consent to processing at any time without affecting the lawfulness of processing, which was made on the basis of the consent expressed prior to its withdrawal. The statement of withdrawal of consent to the processing of personal data requires its submission in writing or by e-mail to the email address iod@dfm.com.pl ;

 

9)    you have the right to lodge a complaint with the supervisory body, competent for the protection of personal data, when you believe that the processing of your personal data violates of the provisions of the GDPR;

 

10) providing your personal data is voluntary, but may be necessary to maintain active business contact;

 

11)       your data provided to DFM will not be processed by automated means, and in particular they will not be profiled.
